Due to the hundreds of enquiries I receive, with most not giving me enough
information to form an opinion as to whether the enquiry is genuine or a time
waster, I have added a few extra compulsory parts to the below form.

Please understand it is from this initial correspondence I have to decide whether
or not a meeting would be value for money for you and acceptable for me.
Remembering I offer my service to genuine gentlemen and expect to be treated
as a lady, a girlfriend and even a lover, so bear that in mind when filling in the form.

Please note:

It is pointless using this form for appointments required in the next 48 hours as it just does not allow adequate time for mails to go backwards and forwards to arrange things properly.

Visits to a hotel will only be considered with a name, contact number,  and details of hotel, then confirmed once you have reservation reference. I will then need
to speak to you at the hotel before I set off to see you.

Visits to your home will require a real name, address and landline phone number that can be verified either
by directory enquiries or other means.
